I could explain that for you. If you keep your cross hair at the center of your screen, it is a head shot at ground level. So if you were above someone, move your cross hair down a bit for a head shot. Same applies if they were above you. And it is easier to head shot combines because their head is larger. If the person is standing still, it is very easy to get a head shot, just point and shoot =\. All of these could explain how Deathsrto moved from behind a wall and head shot someone.

lol a litle correction to ur explanation, the cross hair is alway in the middle of your screen but like u said if you look straight foward then yes that is a ground lvl head shot.

and if your fast with the trigger u can go turning corners and getting head shots. besides its not unheard of to get lucky, ive taken a shotgun double shot to the face and survived (got terrible acne though...). its all about that rabbits foot sometimes!

All models have the same hit-boxes. The model's head is bigger, it makes no difference on the actual hit.
